Rowlands has Alzheimer’s disease, according to the screen legend’s son Nick Cassavetes, who also directed the film.

Actress Gena Rowlands attends the 88th Annual Academy Awards at Hollywood & Highland Center on February 28, 2016 in Hollywood, California. “I got my mom to play older Allie, and we spent a lot of time talking about Alzheimer’s and wanting to be authentic with it,” Cassavetes told Entertainment Weekly while celebrating the film’s 20th anniversary.

The younger Rowlands told O Magazine in 2004 that playing Allie “was particularly hard” because of her personal connection to the disease.
