The B.C. coroner and the Coastal Health Authority are both investigating

Vancouver Coastal Health has launched an internal investigation into the suicide of a teenager while he was in the care of the emergency department at Lions Gate Hospital in North Vancouver.

“We will keep the patient’s family apprised of any recommendations stemming from the review of this tragic case.” According to an individual with knowledge of the tragedy, a 17-year old man on a “suicide watch” was alone in a dimly lit room when he used a piece of medical equipment in the room to asphyxiate himself. The hospital would not confirm that.

The B.C. Coroners Service said it is also is in the early stages of “fact-finding” investigation into the March 23 death.
