School dental program prevents 80 percent of cavities with one-time, non-invasive treatment jamanetworkopen

A stakeholder and community advisory board was created to inform the design and conduct of the trial and assist in the interpretation and dissemination of findings. The board consisted of 35 local health and education leaders, including representatives from the New York City Department of Health, researchers, clinicians, school principals, school nurses, teachers, and parents.

Exclusion criteria for individual participants included any child who did not speak English and children enrolled in special education classrooms. For ethical purposes, any child in an enrolled school was eligible to receive care. However, only children in grades kindergarten through grade 3 were included in the study for analysis because they were expected to remain enrolled in the school at the time of follow-up.

Allocation was performed at the school level and concealed from the potential participants within each school. Randomization was performed by R.R.R. and verified by T.B.-G.Children were randomized at the school level to receive either an experimental condition or standard of care active comparator treatment.

Treatments were provided in a single application after a baseline examination. For the experimental treatment, a single drop of 38% SDF was dispensed into a disposable mixing well and applied as specified for a minimum of 30 seconds. Treated sites were then air dried for a minimum of 60 seconds. For standard of care, a cavity conditioner was applied to pits and fissures for 10 seconds.

Our primary outcomes were the proportion of children with arrested carious lesions and the proportion of children with no cumulative incidence of decayed teeth from previously sound dentition . Arrest failure was recorded if the tooth presented at baseline with untreated caries, received treatment at baseline with either experimental treatment or standard of care, and presented at follow-up with either untreated caries or a filling .

The CALERIE intervention slowed pace of aging measured from participants' blood DNA methylation using the algorithm DunedinPACE (Pace of Aging, Computed from the Epigenome). The intervention effect on DunedinPACE represented a 2-3 percent slowing in the pace of aging, which in other studies translates to a 10-15 percent reduction in mortality risk, an effect similar to a smoking cessation intervention. Over supplementation with vitamin B12 alters microbe-host interactions in the gut leading to accelerated Citrobacter rodentium colonization and pathogenesis in mice - MicrobiomeBackground Vitamin B12 supplements typically contain doses that far exceed the recommended daily amount, and high exposures are generally considered safe. Competitive and syntrophic interactions for B12 exist between microbes in the gut. Yet, to what extent excessive levels contribute to the activities of the gut microbiota remains unclear. The objective of this study was to evaluate the effect of B12 on microbial ecology using a B12 supplemented mouse model with Citrobacter rodentium, a mouse-specific pathogen. Mice were fed a standard chow diet and received either water or water supplemented with B12 (cyanocobalamin: ~120 μg/day), which equates to approximately 25 mg in humans. Infection severity was determined by body weight, pathogen load, and histopathologic scoring. Host biomarkers of inflammation were assessed in the colon before and after the pathogen challenge. Results Cyanocobalamin supplementation enhanced pathogen colonization at day 1 (P | 0.05) and day 3 (P | 0.01) postinfection. The impact of B12 on gut microbial communities, although minor, was distinct and attributed to the changes in the Lachnospiraceae populations and reduced alpha diversity. Cyanocobalamin treatment disrupted the activity of the low-abundance community members of the gut microbiota. It enhanced the amount of interleukin-12 p40 subunit protein (IL12/23p40; P | 0.001) and interleukin-17a (IL-17A; P | 0.05) in the colon of naïve mice. This immune phenotype was microbe dependent, and the response varied based on the baseline microbiota. The cecal metatranscriptome revealed that excessive cyanocobalamin decreased the expression of glucose utilizing genes by C. rodentium, a metabolic attribute previously associated with pathogen virulence. Conclusions Oral vitamin B12 supplementation promoted C. rodentium colonization in mice by altering the activities of the Lachnospiraceae populations in the gut.
