More than 3,000 patients at a surgical center in New Jersey may have been exposed to HIV, hepatitis B and hepatitis C, the New Jersey Department of Health confirmed earlier this week.

I work in sterilization of medical equipment, I can vouch I have seen multiple job offers in the USA who only require workers to have a high school degree, I highly doubt most of them know the importance of the parameters that must be respected. Change must happen.

Kinda makes you wonder if it was “accidental”

That's ridiculous how could something like this happen in 2018. It sounds like negligence. Heads should roll. Hepatitis A &B. Exposure shouldn't had happen.
