Another very important financial implication to patients that may not be readily apparent: in addition to driving accurate care, records are frequently used by life insurance co's to set rates for coverage &/or approve/deny coverage. So many reasons these matter! CC chrissyfarr

It is quite possible that this happened due to a patient matching system used by healthcare organizations called Master Patient Index (MPI). It is a fuzzy match algorithm that sometimes consolidates records if a certain score threshold is met, leading to identity errors.
