'Disturbing' error: SA Health computer glitch adds digit to medication dosages

SA Health is investigating whether patients have been overdosed with medication as a result of a glitch in the computer system used at some of the state's major hospitals.The glitch was duplicating the last digit of medication dosesOn Wednesday night, staff at the Queen Elizabeth Hospital, Royal Adelaide Hospital and Noarlunga Hospital were sent an urgent memo informing them of an issue with the Sunrise computer system which was duplicating the last digit of medication doses.

In a statement, a spokesperson for SA Health said they were not aware of any "adverse clinical outcomes" at this time. "Additional prescription reviews by medical officers, nursing, midwifery and pharmacists are in place while we investigate the root cause of the intermittent issue."Andrew Knox was one of 10 patients underdosed in SA Health's chemotherapy bungle.

"I found it particularly disturbing because the parallel to our underdosing and the failure on this occasion is quite close," he said."Merely sending out an urgent memo to nursing staff to be on the lookout for what appears to be higher dosages is simply no better than what we experienced and what the deputy coroner deplored."

FFS this is the 21st century and you sell software this crappy? Ever thought of adding a parameter that alerts when a dose exceeds a set range? You can do better with MS-DOS on a 1985 PC for crying’ out loud.

A glitch? Wrong input = wrong output... 😖

This is the modern world we live in. Computer control our lives.

Trust computers, yeah nah.

You'd be shocked to know while errors do happen - most nurses tend to question orders asking you to give 100mg of a drug usually given at 1/10th that dose. We're not damn idiots.

And this is why we need humans to check things. The same reason robodebt went wrong and 2000 people ended up dead from it
